What are the Symptoms of Smallpox? Smallpox symptoms: Symptoms includefever, chills, headache, nausea, vomiting and severe muscle aches. This stage usually lasts for 2 to 4 days and on the 4th day the fever drops and the characteristics of the rash begin to appear. The rash starts out as macules, which are flat spots, which then quickly progress to papules, known as raised spots. The papules begin to enlarge and filled with a clear fluid. The fluid then changes to pus. Over time the pustules, pus filled papules, start to form scabs and over time, the dried scabs falls off the skin.
